Thank you for choosing MultiCenter Physical Therapy for your musculo-skeletal rehabilitation. Our goal is to assist your recovery through the use of education, hands on care, and establishing a personalized home program.
In the initial visit, our objective is to assess your musculoskeletal system to determine why you have pain and/or immobility. The examination involves assessing joint and soft tissue mobility, strength, neuromuscular control and flexibility. Prior to starting treatment we explain the findings of the evaluation, discuss treatment goals, and outline the treatment process. A summary letter will then be sent to your referring physician.
Our treatment process is directed towards restoring movement by using manual therapy (joint and soft tissue mobilization), neuromuscular re-education, and strengthening exercises to coordinate correct movement patterns. Modalities such as ultrasound, electrical stimulation, iontophoresis, hot or cold packs may also be used to supplement the treatment. Each patient will receive a home program designed to alleviate pain and compliment the individual treatment sessions. A re-evaluation and final instructions will be given upon the last treatment to attempt to ensure success in reaching patient goals. A final letter will then be sent to your physician.
Please remember that rehabilitation is a process and requires a committed effort by the patient. Your therapist will be happy to answer any questions regarding your treatment.

- Completed Registration and Intake forms:
- Your insurance information and card
- Identification
- Physician referral
And Please Wear:
- Loose comfortable clothes
- Socks and Tennis Shoes (if possible)
